St James' Surgery

REGISTERED DOCTOR

Patients at St James' are not registered with a specific doctor, but with the practice as a whole. However, a patient may express a preference to be seen by a particular doctor. That doctor may refuse to see the patient if they have reasonable grounds so to decline.

NEW PATIENTS

All newly registered patients are requested to complete a questionnaire and make an appointment with the practice nurse or for a “New Patient Check” before seeing their new doctor. Please bring with you evidence of any existing medication prescribed by your former GP. You will also be asked to bring a sample of urine with you.
This check helps to identify any repeat medication you are already taking and helps us set up the arrangements to continue it. Also we can establish any medical conditions you may have that need us to take action.

CARERS

The surgery holds a register of carers. If you are a carer and would like you to add your name to our register please ask reception for a carer's registration form. A carer is an individual, whatever their age, who provides or supervises a substantial amount of care on a regular basis to a child, relative, partner or neighbour who is unable to manage on their own due to illness, disabillity, frailty, mental distress or impairment. A carer can be a child looking after an older person, parent or sibling, or an older person looking after a disabled partner or child. A parent of a disabled child is both a parent and a carer.
